Eligibility Requirements

Who Can Apply

Patient passed from ALS

The assistance is for individuals who have passed away from Amyotrophic Lateral Sclerosis (ALS). A confirmed ALS diagnosis is required.

Applicant is next of kin or executor

The person applying must be the spouse, parent, child, sibling, or legally appointed executor of the deceased's estate.

Doctor's note confirming ALS diagnosis

We require documentation from a licensed physician confirming the ALS diagnosis. This protects the integrity of the program for all families we serve.

Payment goes directly to the funeral home

Funds are disbursed directly to the funeral home or burial vendor — never as cash to the applicant. This ensures the funds are used as intended.

Documented financial need

We ask applicants to briefly describe their financial situation. This program is designed for families with genuine need.
